The Oak Counseling & Psychiatric Services is located at Three Forest Plaza Building, 12221 Merit Drive, Suite 450, Dallas, TX 75251. Our hours are Monday – Friday from 8:00am – 5:00pm and closed from 12:00 pm – 1:00pm for lunch.
Patients are seen by appointment only. To schedule an appointment, please call (972)770-1032 during regular business hours. Should you need to cancel or reschedule an appointment, we ask for 24 hours’ notice. If three (3) or more appointments are missed without notifying us of cancelation (example: no show or same day cancelation), you are subject to being discharged from our clinic upon review and approval from your provider.
New Patients
New patients are asked to arrive 30 minutes early to complete paperwork. If your paperwork is not completed prior to your appointment, you could be asked to reschedule your appointment or your appointment time will be changed due to availability on the schedule for that day.
Billing
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
Refill Requests
All requests for prescription refills must be made 48 hours in advance. You must have your pharmacy fax us for your refill request. Please do not call Medical City Green Oaks Hospital for refills, they will not be able to assist you.
After-Hours & Emergencies
If you are experiencing a medical emergency, dial 911 or go to the nearest emergency room.
